Mental health care for older people

The Community Mental Health Team for Older People (CMHT OP)  is an integrated mental health and social care team providing assessment, intervention and treatment. The CMHT works in partnership with service users, over the age of 65 years, and their carers to promote recovery.

The team is multi-disciplinary and staffed by community mental health nurses, social workers, occupational therapists, psychologists, psychiatrists, support workers and admin staff. 

The team also works in partnership with the Diagnostic Memory Clinic, Adult Community Mental Health Teams, Tower Hamlets Adult Social Care, GPs, Alzheimer’s Society and other agencies, including the borough’s residential and nursing homes.

Diagnostic Memory Clinic (DMC)

The Diagnostic Memory Clinic (DMC)  is a multi-disciplinary team consisting of:

  • psychiatrists
  • psychologists, and
  • dementia nurse specialists. 

The team also has direct access to occupational therapists and mental health social workers. 

The team provides a comprehensive assessment for people, of any age, who are experiencing memory problems and other cognitive difficulties to determine if they have symptoms of dementia.  Where appropriate, the team are also able to offer a social care needs and carer's assessment. 

The team works actively in partnership with service users, carers and relatives to empower them to plan their care following diagnosis and maintain a quality of life.

This includes access to evidence-based interventions, such as :

  • Cognitive Stimulation Therapy
  • medication to alleviate some of the symptoms of dementia, and
  • an opportunity to get involved in research.
