Social prescribers
Social prescribing provides vital support and care to help Tower Hamlets residents improve their health and wellbeing without medical intervention.
Often, patients visit their GP for reasons other than clinical problems, as they might not know where to access support for wider social issues such as welfare advice, isolation or housing problems that we know can negatively impact their overall health and wellbeing.
Social prescribers can give information about the free and low cost services around:
- financial support and advice
- employment, training and volunteering
- education and learning
- healthy lifestyle advice and physical activity
- arts, music, outdoors and creative activities
- befriending, counselling and emotional support groups.
Social prescribing in Tower Hamlets
The Tower Hamlets Social Prescribing Service, run by the GP Care Group, aims to address the social factors affecting health and wellbeing by connecting local residents to non-medical support available within the borough.
The service’s key aims are to prevent the development of associated complications with residents’ health and wellbeing, empower residents to play an active role in their health and wellbeing, and also work with residents and healthcare providers to plan suitable interventions.
