What is reablement
Reablement is a short-term support service that can work with you to regain your skills or ability to manage everyday tasks and to live as independently as possible.
The Reablement Service is an occupational therapy-led team with an emphasis on rehabilitation and enablement.
How to access reablement
Reablement sits within Adult Social Care. There is no direct access into reablement. Tower Hamlets residents can access reablement by contacting Tower Hamlets Connect to discuss their needs.
Why you may need reablement
You might have lost your confidence or ability to do things for yourself because of:
- an accident
- ill health
- a disability
- a stay in hospital.
If you have recently started to find difficulty with everyday tasks, either inside or outside of the home, we can support you to regain your independence and do things for yourself again.
Who is eligible?
You can work with reablement if you:
• are over 18 years old
• live in Tower Hamlets
• have recently lost some of your skills confidence or independence.
How reablement works
Occupational therapists (OTs) work with you for up to six weeks to progress independence with the daily activities that are important to you or that you need to complete.
To support this, the reablement team has its own in-house reablement officers (ROs). They provide day to day to support under the direction of the OTs, to encourage and support you to carry out tasks yourself or with you, rather than carrying out tasks for you.
The Reablement Team also work closely with our health partners. If you are receiving rehabilitation from a community health team, an OT from either team can work with you to achieve your goals.
What kind of tasks are supported
We will discuss what tasks you need support with. We will then agree a plan - your reablement programme - detailing how we will work together to support you to get better at managing these tasks for yourself.
Your reablement programme will be tailored to your individual needs, and the level of support we provide will change as you progress in meeting your goals.
How long can I get support for?
The length of your reablement programme depends on your individual circumstances and how you progress in meeting your goals.
A programme will usually last up to six weeks, at longest. After this time your situation will be reviewed.
What happens if I still need support after reablement?
If you still need ongoing support after reablement, we will discuss your options with you to find out:
- whether you would qualify for ongoing support
- how we can help you further.
