Online carers self-assessment
Welcome to the Tower Hamlets Connect Carers Self-Assessment Form
Developed with unpaid carers, this online carers self-assessment form is designed to support you with your needs as an unpaid carer. It is completely separate from any assessment/s that may be carried out for the person you care for.
You can use this form to help you identify areas where you require more support and find tools and resources to help you.
Before you begin, it is recommended that you read the section on carers assessments (Carer's assessment | Tower Hamlets Connect).
The form typically takes around 20 minutes to complete, but could take longer depending on your circumstances. There is no option to save and return to the form at a later date, so you will need to complete and submit in one session. You may wish to download and complete the Word version of the form (stage 2 only) and copy and paste your responses into the online form once you are ready.

Who is the form for?
This online carers self-assessment form is for adults (aged 18+) who are providing unpaid care to another adult or adults who lives in Tower Hamlets. If you are under 18, or providing unpaid care for someone under the age of 18 a different carers assessment is required which will need to be arranged by the Multi Agency Support Team (MAST) by calling 020 7364 5006 or emailing MAST@towerhamlets.gov.uk. Stage 1: self-help information and advice
The first stage of the self-assessment will be a series of categories and questions to identify the areas where you require support. At the end of this stage, you will be presented with links and resources for self-help which you will be able to email to yourself so you can come back to the information provided at any time.
Stage 2: if further support is requiredÂ
Following this if you do feel that you still need support, you can continue by answering a series of further questions about your needs. This will become a full carers assessment. On completion, an advocate from Carers Centre Tower Hamlets (the Carers Centre) will contact you within two weeks to discuss your needs as an unpaid carer in more detail.
